Affiliation Agreements
Many departments and schools require or encourage students to participate in clinical or field training as a part of their program curriculum. To coordinate these student placements, an affiliation agreement must be in place between the facility and Purdue University. All campuses should follow the procedure below to initiate an affiliation agreement.
When a program or school wishes to engage with a facility, allowing their student(s) to obtain training, the department or program representative shall:
- Download and complete one of the Purdue standard affiliation agreement templates, found in the OLC Intake Portal Knowledge Base.
- Present the standard agreement to the facility for review.
- If the facility accepts the standard agreement with no revisions, please have the facility sign the agreement and submit the signed agreement to OLC through a “Contract Request” in the OLC Intake Portal.
- If the facility requests revisions or would rather use their own agreement, please submit the redlined template or facility agreement to OLC through a “Contract Request” in the OLC Intake Portal. After submission, OLC will negotiate the terms of the agreement directly with the facility.
- Once the terms have been approved or a signed standard agreement is received, OLC will route the agreement for signature. A copy of the signed agreement will be sent to the department representative.
- Present the standard agreement to the facility for review.
When submitting a Contract Request, please be sure to include all important information to avoid unnecessary delays, especially:
- Student placement date
- Facility or site contact, including name and email
- Facility or site signatory information, including email address
